New Patient Registration

                    

To register as a new patient, please call our office first. We will ask you a few general questions, please have your insurance card ready. This process will take about 10 minutes. Please bring your insurance card and photo identification (driver's license or I.D.) to your appointment.

You will be given forms which may include:

  • Consent to Treat and Financial Agreement
  • HIPAA (privacy form)
  • Health History
  • Medicare COB (Medicare patients only)
